If an inmate has been in jail since 2004, and they were in Minimum or Median, and now in a Maximum secuirty prison, what does that usually mean? Does it mean they become violent or what?

I think you pretty much hit the nail on the head. To be moved to a higher degree of security, one has either become a bigger threat by being involved in activities that present a risk, or the individual has serious issues with complying with the established rules placing him/her in a higher risk category!
