I doubt you will find any template already suited to something so specific. You're best bet is to review relevant case law or get an attorney.
I'm all for handling your own legal affairs without an attorney. However, such an undertaking requires alot of reading and research.

Theres no excuse in the world that should get you a reduced SENTENCE for drink driving, its the most preventable thing going...if you get a good sentence, then you deserve it and not sympathy, you put the glass to your lips and took the drink and then drove a car, no one else, and you cannot dispute the results of blood and urine tests, take whats coming and learn from it

If you know that you are guilty you should so plead. The sentences are mandatory, but the judge has some discretion and a guilty plea will mitigate the sentence. Pleading not guilty knowing the opposite to be true is likely to count against you.
