When they look in your eye they are looking at the retina (the back of your eye that contains the rods and cones), the optic nerve head (the optic nerve sends the information about what you see back to your brain), the macula (this is what you use to look directly at things), and the blood vessels.
The retina is considered by some to be part of the brain so they are looking at the "brain" but not in the way that most people would think of it.
They are looking at these structures to check for signs of glaucoma, macular degeneration, retinal holes or tears or detachments, and numerous other problems.

When the doctor uses an ophthalmoscope to look in the eye, they are looking at the retinal vessels inside, the fundus, and other things which may contribute to the patient's diagnosis. No, they are not looking at your brain.

When the doctor looks into you eye with the biomicroscope they are actually seeing the cornea, iris, conjunctiva, retina, optic nerve and macula. They are looking for signs of cataracts, macular degeneration, diabetic eye disease and corneal ulcers. They dont look at your brain!

are you serious? They look to see how your eye reacts to the light. They can tell a lot. Like if you high on something and if your pupils don't react right they know somethings wrong.
