ER.
I think it wins just because its been on so long and has stayed good for almost the same amount of time.
Greys Anatomy seems ok from all the press it gets lately about racist actors and other actors wanting pay raises. Now, some actors are leaving for a possible spin off. So, its just too early to award anything to GA.
Scrubs, from what I've seen, is too much comedy to be considered a true medical drama.
House......interesting choice there. I think much of the show's hype is carried by the main actor. I can't really tell you anything about any of the other actors. Compare that to ER and is long list of actors who've starred on the show from George Clooney, Anthony Edwards and Noah Wylie. Not too mention ER has had a ton of guest starring roles from all kinds of people.

I would pick between House and Grey's Anatomy. It's a tough choice, but it depends on what you enjoy watching. They are both great at what and who they target. For example, Grey's targets teenagers (primarily, although others do watch), and they are perfect at capturing their interest and having interesting situations.
House on the other hand is for a perhaps older audience. It focuses on sarcasm as it's main topic, and everything medical can just flow around Houses' sarcastic nature. Here we find much more depth and less of the soap-opera quality, but it is no more the better than Grey's Anatomy, even though it does show a different side to medical dramas.
I would have to, for now, leave it as a tie and see where the shows go from here. I noticed that recently the Grey's Anatomy episodes have become more intriguing to older audiences as well, which is a big step. We'll have to see where the shows go now, and then perhaps in the future decide which is the better.

ER! I have never watched House, it must be on when I'm watching something else. Greys Anatomy I've watched a few times recently, just to see what its all about......and while I get the whole McDreamy thing....thats about it.....It just all felt contrived to me somehow (sorry GA fans!)
As for Scrubs.....I'm sorry....but to even think that some of what happens on that show, could even happen in real life....is S-C-A-R-E-Y!!!! Not funny :(

Oh no. I shouldn't have clicked on this question LOL.
This is hard! I'm obsessed with House and Grey's Anatomy and can't pick from those two. I love them both! Their both amazing shows with a great cast. Grey's has all that sappy love drama going on in it which just gets you hooked and then you've got House which stars Hugh Laurie, who is just amazing on that show. So, I really can't decide. I love them both!

House is my favorite show and I just want to say two things here. Hugh Laurie (check this week's t.v. guide) mentions the fact regarding it's an ensemble cast and the name makes it seem to center on Dr. House. And as far as guest appearances, House is getting better. The most recent episode featured Dave Matthews as the patient and Kurtwood Smith (Red Foreman, That 70's Show) as his father.
