A four month old will be considered a lap child. Just be prepared and have everything you might need with you. A pacifier, or something to suckle, would be a good thing to bring along in case the baby has pressure on their ears. It isn't easy to travel with such a young child so patience is a virtue.

Kids up to 3 can be a lap child. I am not sure who makes the crazy rule. . . but you have to sit in your seat in the full upright position, and your lap belt has to be tight and secure. . .but you can hold the infant in your lap without the seat belt. . . .
Oh yeah, if you are taking a child, 3 you have to buy a ticket, and you can bring your car seat from home. . .but you have to make sure that it fits the flight standards. . . . .
Here is a little tip that might make it easier ( I used this countless time with my kids) Keep your child in your car seat. Go through security, get to the terminal, right before you board, ask the airline attendant at the gate if there are any extra seats, if there are, you might be able to get moved right next to it. If you are able to get moved to the open seat you will be able to take the seat and put your little one in the car seat, in that empty spot. Worst case of not doing this is that you will have to hold the baby for the whole flight, as for the car seat, you can gate check it. You have nothing to lose by doing this, only a possible free seat for your little one.
Also, I always fly with Frontier, they have mini TVs on the seat backs, even thought your little one will not be able to listen with headsets, I have found this to be a HUGE help in entertaining little ones during a flight.

A 4 month old will see on an adults lap, they belt and then place the child on their lap. During take off and landing, place a pacifier or bottle in the babies mouth. This helps the pressure in the babies ears. Otherwise, it can be painful. (chewing gum same thing for adults)

Sadly yes an infant sits on your lap. From what I've seen they are not restrained at all except by the parents holding them. At least that's how it was on my 2 hour flight with Southwest
