Have you tried meditation before? It helps you to focus. I don't believe it has anything to do with being bipolar or schizophrenia.
The mind is very very powerful. It maybe something that your subconscious wants you to pay attention to. Instead of blocking those "irrelevant" thoughts, pay more attention to them. It could be something that needs to be done or paid attention to anyway.

gee, phd p, I didn't know that schizophrenia could be that easily diagnosed over the internet... actually... YOU CANT!!!
I checked your profile and you have been diagnosing everybody w schizophrenia or bipolar, even a girl whose question was about a small lump in her eye....????
Anyway, going back to the original question... what works for me is a cup of coke or anything w caffeine in it (I don't drink coffee, but I guess that would help too). Before studying for a long time, try relaxing a little before starting. Think of your favorite color or favorite smell, close your eyes and breathe deeply for a few seconds. Sometimes, if you are too stressed out, is simply too difficult to block those thoughts. They sux..... Every person is different and you need to try different things until you find what works for you.
good luck in finding what works for you

The trick is not to block the random thoughts it is to notice and then ignore it. If you are doing something boring but necessary do a short stint take a break then pick up where you left off. The more effort you put into worrying and fighting the harder it will be to concentrate. "relax" avoid all drugs and quack shrinks

I am a trained Psychiatrist.
From what you describe, I have a pretty good idea that what you are suffering from is Schizophrenia and possibly Bipolar II.
You have a chemical imbalance that needs to be corrected. You have elevated seratonin and dopamine levels. That is what is making you act this way.
You need to see a psychiatrist immediatly. Ask him to put you on Respiridone and Lithium.
Once you are properly diagnosed you need to inform the motor vehicle branch and let them know that you are potentialy a dangerous driver because of a mental illness.
Trust me... I have been doing this for 25 years. I know schizophrenia when I see schizophrenia and I am sorry to say.... you have it!
