the introduction comes first, the thesis sentence is the last sentence of the introduction.
Sorry don't know abot 1-5.

Well I'm not sure about the questions-sorry.
But there are three parts to an introduction:
1) Hook/Grabber-to get the reader's attention
2) Introduce your topic
3) Thesis- 3 parts to the thesis- Tell what each body paragraph is going to be about
