The female mosquitoes in the genus Anopheles, are responsible for the most human deaths. These insects are the zoonotic vector of Malaria (plasmodium) which has and still does kill more people than any war, tsunami, famine or other disease ever has!

I'd guess Homo sapiens may have the edge on the mosquito. It does kind of depend on the meaning of "cause of death." If a political regime relocates millions of people and they starve, has the political leader directly caused the other person's death?
One thing we can probably say for sure is that it's not the hippo. There's a bit of urban legend mixed with a few educated guesses that suggest hippos are responsible for more human deaths than any other wild African vertebrate, and that mixture has often been cited as fact on the internet, but I can't find any scientific documentation of it. Regardless, that estimate categorically excludes dogs, mosquitoes, and other less exotic killers.

All the land animals fall far behind the mosquito, tse tse fly and many other insects that carry parasites. But it is not the mosquito or the flies that do the killing they only deliver the parasite!
It depends how far you want to go in terms of defining animal but the biggest and most efficient killers are the viruses and many pathogens that are life forms that still kill thousands of people every year.

I think it's the humble fly. I believe that it's carried a fair amount of disease. I don't think it means to, but it will insist on vomiting all over peoples food and leaving it's germ infested eggs everywhere including in some cases, under peoples skin. OK I do know that it does cure gangrene and other pustules, but for a small creature, it manages to carry more vile pestilence per square inch than most.

Hippos ,have killed more people than anything else in Africa
because of shrinking Habitats because of dried up rivers and lakes and expanding civilization
and they are very bad tempered and territotial.
bacteria and virus are not animals ,
but humans are and they would win this by billions
