Some of them wear "clear" underwear (method used in the Notebook even though that was a PG13 film but the crew wanted it to be much more graphic than it was) and others have body parts digitally removed (Underworld Evolution for example).
Actual intercourse only occurs in porn. Actors talk about all the time how filming sex scenes is very uncomfortable for everyone involved (again Underworld Evolution where the director and lead actress were married).

No! How naive are you?
First, you have the problem of being surrounded by a crew of like 50-100 people, and then having to do it on command. (This is why being a porn star ain't easy.) Also, they probably have to do several takes with set-ups from different angles.
Secondly, just because two people are acting together - which is their job - doesn't mean they'd actually want to boink. Usually the actors involved have spouses of significant others, and just because they're asked to film a sex scene doesn't meant they instantly get to cheat.
And thirdly, actual intercourse would doubtless earn a film an automatic NC-17 rating.
Sex scenes are just cleverly filmed. The actors usually wear little patches over their gentials, even.

No, I know a guy who performed in a b-movie, and they used a towel. In fact, he said that the actress used all these extra precautions lol
There was a recent movie where an actress from New Zealand (married too) performed a scene that show a brief few seconds of her taking him in her mouth. This was a mainstream R movie too.

Ummm, yeah. It's a perk to be an actor. Having intercourse with all the female stars. So, ummm, go take some acting lessons, and try to star in a film with Angelina Jolee or something.

Usually not, that kinda stuff would cost too much, especially for big name actors and actresses. Occasionally there are movies that have unsimulated sex, but usually it's a big enough deal you know in advance.
