It has two meanings, depending on context.
In gay male couples, the one doing the penetrating is the top, and the one being penetrated is the bottom.
In a more general sense, top can mean the more dominant partner and bottom the more submissive.

Top is the boy who is the boy, bottom is the boy who is the girl. Top does it puts it into the other boy, bottom gets it in the bum from the other boy.
