my old job did the same thing...i told them if they did not give it to me i was going to contact tax franchise board they will actually instigate them??? and soon as i said that he went to his desk and gave it to me they gave it to me i think he wanted to do his taxes late and did not wanted to get tax early.

Contact the Irs and provide them with your employer's name, address and telephone number and if known your employer's identification number or EIN. The IRS will have you complete Form 4852 which is a substitute for the W-2 and you will have to use your last pay stub to fill the form out.

cal IRS and report them. amke sure you have a paycheck stub to prove you worked thier and how much they took out of your pay
