Princess Toadstool is generally referred to as one of three different things: Peach, Princess Peach, and Princess Toadstool. Thus, the most logical conclusion is that she is a princess whose name is Peach Toadstool. This is backed up further by her parents who are King Toadstool and Queen Toadstool.

In Japan, the Princess's name has always been Peach, but when Super Mario Bros was released in America, Nintendo of America decided to rename her Princess Toadstool, probably because she was the princess of the Mushroom Kingdom, and a "toadstool" is a type of mushroom.

It's because Japanese are perverts. The peach has long been a representative of the female in a sexual context, owing to its superficial resemblance to the enternal appearance of female genetalia. Since Japanese are perverts, it's natural that they refer to women in this way. They aren't the first to do it, and probably won't be the last.

Americans, on the other hand, are more God-fearing, so that kind of lewd garbage won't fly here. So when the Japanese started selling their games here, they changed the name when they translated it. Simple.

It could have been worse. They might have given her a monstrous chest and virtually no clothing as so many of their other depictions of women have. Perhaps they lacked the graphics technology to do this at the time.
