You might try some meditation exercises to learn how to slow down and relax. Imagine a peaceful place, exhale slowly, visualize a lovely scene.
I'm not an expert nor a teacher but hopefully you get the idea.

Eating is important - its fuel for you body the same as gas is for a car.
If you're asking if you lack confidence then you probably do.

Here's some techniques to help you.
First - think about the last exam you took. What happened? What worked for you - what didn't? This will help you change your behavior.

What you're trying to do is predict what questions will be on the exam so you can make sure you have the right answers.

Begin review for an exam at least one week before the test.

Here's how to review:
Reach chapter Objectives and the Summary to check what you do and don't know. Put check marks beside everything you do know. That'll help you feel more confident right away.

Study what you don't know.
Review any notes or previous tests, quizzes, exams you've taken in the class to help your not only remember but predict what kind of questions will be on the next one.
Read your course outline if you have one.

The night before the exam, eat well, do some visualization (just close your eyes, relax and see yourself taking the test and being relaxed and knowing all the correct answers - this is rehearsal, ok?) Then while you're nice and relaxed - go to bed, get some rest.

Day of the exam:
Arrive early, review your notes, take 3 deep slow breaths during which you inhale deeply and exhale completely.
Once you've done this you are calm and will be able to focus.
When you are given the test - listen carefully to what you teacher says.
Put your NAME on the test. Then write any notes you are afraid you might forget as you get to taking the test in the margins or on the back of the test.
Take another deep breath and as you inhale remind yourself that you do know the answers. Exhale slowly and begin.
Read each question carefully. Answer it if you can and move on.

If you come to a question you don't know the answer to put a mark beside it and move on, Answer every question you can the first time through. This gives you confidence and sometimes jogs your memory for answers you haven't recalled yet.

Go back and answer the questions you passed over the first time.

Be sure you answer EVERY question unless there is a penalty for wrong answers beyond just loss of points. Remember - you've got at least a 50% chance of giving the right answer.

DON'T change an answer once you have put one down unless you are absolutely postive that the first answer you gave is incorrect. Trust your instincts.

Since you probably study at home, there's no need to skip your lunch period to just study. It is better for you go grab something to eat and then just read over your notes for about 10 minutes. You have to be confident in yourself and 95% is really good. You're getting straight A's according to what I read so there's no need to freak out. Just relax and you will do great. Also try to add in some entertaining activities like going to the mall or to the movies...you deserve it for being an 'A' student.
I'm now in college and I graduated high school with a 3.98 GPA. I didn't miss lunch for nothing or else I would be worrying about the noises my stomach would be making during the test. Relax, eat, and boost your confidence. You'll be just fine. May God be with you in your studies
Be blessed.

Test taking tips: Over study up to the evening before the test, then get a good nights sleep, eat on schedule. It sounds like you're "cramming" and then letting your nerves rull you and you can't handle it. AGAIN, study long before the test, not the night before or the day of. Then relax and enjoy the test because you know the answers.
