There are hundreds of billions of nerve cells in the brain with trillions and trillions of connections through the synapses. As yet there is no electronic chip with more than a few million transistors on it so, no, it certainly is not possible at this time. Perhaps in the far future.

It is certainly possible for ALL of the data in a human brain to fit on a digital form of media(hard drive, disc, etc.), but the means of transferring the data is a long time away. Right now there is no way of organizing brain matter into a contiguous string of 1's and 0's and collecting every thought that has been retained. The brain is still something that is yet to be understood in terms of how information is stored and in what way is information retrieved or logged. We know there are electrical signals that are stimulated in certain areas by certain stimuli, but after that it is a big mystery.

You are speaking of some thing in our far distant future, perhaps thousands of years. We at this time understand only a fraction of how the human brain works. It is an extremely complicated machine. Consider a car engine. The pistons the carberator, the exhaust system, the ignition system, the transmission. The human brain is so much more complicated with millions of chemical reactions taking place simultaneously. You are asking for the impossible. Maybe in the movies, but not in the real world, at least until great advances in science have been discovered.
