For my church we basically decided on the typical Black skirt/pants and white long sleeved blouse/ shirt so that the ushers could purchase them from where ever they usually get their clothes.
You can also order them online or pick a pattern from a fabric store and have them made.

i've got not seen an usher uniform. i don't be attentive to of a church that makes use of them. My church in simple terms has a tag on their pocket to show that individual is an usher and could help people if needed.
