With http://kall8.tollfree2.com it costs $2 to setup a number and costs $2 a month and 6.9 cents a minute. Pretty good rates if you don't know how many mintues you are going to use because there are no minimums. It comes with many online features to customize the number. For example you can have the 800 show up on the caller id of your phone so you can identify 800 number callers. Particualry useful if you combine personal and business calls on one phone. Also it comes with its own free voicemail which can answer before your personal voicemail in the case you combine calls on one phone, and the number can be customized to ring based on time of day, days of week, etc.

They're about $4/month with a per minute charge. This is anywhere from 5 cents-20 cents depending on where you go. OR, you can get a 800 number with a VOIP line for about $7-8, and if the calls go to the VOIP line, there is no per minute charge. :-)

I'm not sure what the actual set up fee is, but my conference call number costs about $3 a month.
