Most likely a pulled muscle.

There are any number of things which could cause pain in the lower left side: diverticulitis, hernia, prostate cancer, colon cancer, kidney stone, kidney cancer, constipation, pulled muscle, pinched nerve in his back, pancreatitis, aneurysm, and probably a whole bunch of other things.
The pain with breathing in makes it sound like a pulled muscle and if this started while exercising, then maybe.
Pain, bleeding, or nausea is important enough to see a Dr.

if there are any other symptoms, like numbness, nausea, tingling, call the ambulence..
it could be a pulled muscle, when did it start? how long has he had it?
if it stays the same, no other symptoms, doesn't get worse and you are still worried, a trip to the doctor is not a bad idea..
good luck

Click on the link below, it's a pain and symptoms chart to see if his symptoms match any on the list. Sure hope he feels better but if it hurts that bad, consider taking him to the emergency room.
