Actually, it's probably more than 2,000. But you said it yourself, he's insane. This is one of the key elements in the Joker-Batman story. Batman struggles with the fact that if he killed Joker he would be saving a lot of lives, but then he would be as bad as the Joker. What gives Batman the right to be an executioner? Even after the Joker killed the second Robin, Batman couldn't bring himself to kill him. It's a moral dilemma and it makes the dynamic between the 2 characters very interesting.
If you read the comic called "The Dark Knight Returns", which is set some amount of years in the future(Batman is in his late sixties, early seventies), you get to see Joker finally get what is coming to him.

Even Dick Tracy, the old man of the comics would have shot the Joker like the dog he is (and don't even start me on the Punisher).
