Now would be a good time to back up each accounts data.

Run the following:

Update your virus protection and run a full scan.

If for any scan something can not be removed, repeat the scan in safe mode with the internet line disconnected.

Run "check disk" check both options and restart.
Run Disk Cleanup then Disk Defragment.

I've encountered the same situation before. Until you find out what caused "log off" command to be lost, you can log off that user another way.

Ctrl+Alt+Del will bring up the "Task Manager" program. You can safely log off there, by using the "Shut Down" menu.

I eventually used the "System Restore" utility to restore the lost command. XP Users Only of course.

Try using the buttons Ctrl+Alt+Delete. Press them all at the same time. Then this little menu should come up and somewhere on that menu it will say Shut Down click on it and see if it works like that.
