Putting a condom on an uncircumcised penis is a little different than on a circumcised one. The main thing is to pull back your foreskin all the way when erect before putting on the condom. Then you just put the condom as instructed.
To get the best coverage, when you roll the condom back over the foreskin, pull the foreskin forward towards the head a little and continue to roll the condom down towards the base. It's a little hard to describe, it's like you cover a little, pull forward, cover more, pull forward a little, etc; this way your entire foreskin and most/all of the shaft should be covered.
Usually this is all you'll need to do, and the foreskin kind of stays back and/or bunched up (especially if the foreskin is long) either at the end of the condom (at the base of the penis) or underneathe it. For many guys, the foreskin will roll back over itself and take some of the condom with it, so it kind of double backs.
Basically, if you've troubles putting on a condom, take a few condoms and experiment and practice putting it on yourself until you get it right. The somewhat difficult to describe advice above might help a little.

When not stiff, pull foreskin back & when it is stiff put on condon. For extra pleasure get help from your partner, she will enjoy to participate - it is a good foreplay.

Man, if it's causing you that much trouble I would say spend the ~$3000 to get that fixed. Go for a circumcision. You probably can't clean that well either then. Best of luck to you.
