The focus is half the radius of curvature so the focus is 6 cm.
The distance of object is 23 cm.

1/f=1/do+1/di
1/6=1/23+1/di
di=+8.11 cm

hi/ho=-di/do
hi/5=-8.11/23
hi=-1.76 cm

Your answers are:
