Go to Pet smart, they have a machine in every store that will engrave whatever you want on the tag.

Some stores, like Wal-Mart have a tag engraver by the games, or try a military surplus store, ask your local vet, animal control officer, or shelter worker.

you can order them from places like http://www.pettags.com/ or http://www.terrificpets.com/pet_id_tags/ as a few options or most places like Petsmart have little machines that you put in your money and get one immediately. Those are just a few options. Hope this helps

go to your local petsmart or petco, there should be a machine by the exit that makes dog tags. they are usaully 4.95 or so.
