a gift certificate to a spa for the mom is as much as a gift for the baby as it is for the mom. On the gift certificate, you can attach a coupon (that you make up) for a day of babysitting (so that she can go to the spa).
If you dont want to do that, get some diapers in large or xl. Some large or xl onesies. Not sure what your budget is but some EINSTIEN videos will be something that she could use for way longer than most of the other gifts and babies can start watching them @ about 4 or so months and it gives mom a chance to put a load of clothing in the washer or dryer.
No one else will buy a baby gate, she might need one of those too.... but the spa thing will have the WOO factor!
I'd say go with that and the coupon for the babysitting is a win win for all three of you...your SIL will get the chance to get out of the house, you get to bond with the baby and vice versa! If you have teenage kids they could give a babysitting coupon too

Baby showers can be really hard to buy for. You want to buy something that no one else has thought of, or at least I usually do. Ask your friend what they are decorating the nursery in, you could always try to stick with the theme. Diapers and baby wipes are always nice, because you know they are going to need/use them. With diapers though, stay away from newborn diapers. Lots of people will get newborn and honestly babies don't weat that size for very long. Get size 1 or 2. They will be greatful. Also, the baby medicine chests that they have (contains baby tylenol, gas drops, thermometer, etc) are always nice too. I also think gift cards are nice. That way, when the baby shower is over, if there was something that they needed for baby, but didn't get, they could get it with gift cards given to them.

Give her things others won't buy. Whenever I go to a baby shower I always get a huge bag a fill it with some things like diapers, diaper bag, pacifiers, bottles, teething ring, clothes that are one or two sizes bigger, bigger bibs, potty seat, bowl and spoon set, socks,sippy cup. People always want to buy things that the baby can only use for the first few weeks or so. If you get something that the baby will have to grow into it will be a huge hit as well as a huge help when the time comes to use it. Good Luck

If she's registered, get her something off of there. That's what she is asking for, so it will be guaranteed to be exactly what she wants. It's nice to get baby clothes and stuff, but if she really needs or wants something, it's on the registry. I have a closet full of cute clothes, but I am having to buy some of the stuff I really need myself. Not that I'm complaining about getting gifts, but I'd rather have the stuff off of my registry!
My mom always likes to go to registries after they have been picked over, and there isn't much left. She'll buy all the little items, and put them in a big basket. So if there's pacifiers, bottle nipples, diapers, baby shampoo, etc, you can get tons of little cheap things and make it into one big, really nice gift.

dont waste your money on getting the baby clothes. leave that for the parents to buy, specially if they are picky like i am. i had a huge babyshower, im greatful for the thought, and although the clothes were cute, half of it wasnt even worn, you would be surprised how fast they outgrow outfits. get her something that will really come in handy...my best gifts were the giftcards. my baby is 3 months old and i have yet to pay for formula or diapers out of my own pocket. i even bought a swing, because the swing i was given sucked, it was cheap and had bad reviews. hope this helps...good luck!

find out what she already has but I would get those bottles with the liners (playtex) it doesn't matter how many people buy bottles you can never have enough trust me they are always getting lost etc. those new drop in liners they have are wonderful too but the cheaper baggy style one will suffice (Experience), and if you want to get her a big item like a bassinet just ask if she already has one just ask her what she needs yet, make sure you tell her a spending limit though otherwise she may expect something more than you can afford. It's also nice to get the expecting mother a small gift (not neccesary but appreciated)..........good luck

Avoid the obvious!!! She probably doesn't need another dress, a stuffed animal, a bottle, pacifier, bottle brush and the like. Everyone gives those things. I think (if she is planning on bottlefeeding) a bottlewarmer you plug into your car light is handy, the crib pads that sound an alarm if no breathing is detected is reassuring, a sling to carry baby on her chest is a lifesaver....Just try to find something unique, that you know everyone else is not bringing.

Buy her something she could really use that will save her money.
Bouncer or swing.
The car seat.
The bedding set, These things are pricey but are a huge help and something that is truly needed.
Or you could go with the bath tub and fill it up with:
lotion, shampoo, cotton balls, wash rags, hooded towel etc...
Or buy a diaper bag and fill it with:
out fit, wipes, safety kit...
I'm due in a few weeks and i know some things i could really use are clothes, socks, blankets, diapers, wipes, the everyday essentials.

Vicks cool mist humidifer is a great gift! You never think to get one unless baby is sick or congested but we use ours every night for "white noise" to help my son sleep plus the added benifit of clearing up congestion and keeping nasal passages clean.
My son HATED having his nose sucked out with an asperater and after we started using the humidifer every night we never had to do it again.
They run about $30
