Th real story was of a boy, but his head didn't spin a round and all that 'scary' stuff didn't happen like objects flying and his head spinning and the devil speaking out of him. He was just a eal sick boy and everyone thought the devil was in him.

The Exorcist is a horror novel written by William Peter Blatty first published in 1971. It is based on a supposedly genuine 1949 exorcism Blatty heard about while he was a student in the class of 1950 at Georgetown University, a Jesuit and Catholic school. The exorcism was partially performed in both Cottage City, Maryland [1] and Bel-Nor, Missouri. [2] Several area newspapers reported on a speech a minister gave to an amateur parapsychology society, in which he claimed to have exorcised a demon from a thirteen-year-old boy named Robbie, and that the ordeal lasted a little more than six weeks, ending on April 19, 1949.

It was based on a real exorcism of a little boy. Whether or not he was possessed, no medical treatment helped him. The child's real name has never been released. Catholics recognize the Rite of Exorcism as a legitimate part of the faith.

it was based on a book of same name. It is said the book is based on a a reali life exorcism that took place in the 50s in US and which the author heard a lot about while in his catholic school. The act of exorcism is certainly not fiction but the catholic will probably never admit to its practice.
