Call your network provider, or report it stolen to the police. Give the police your IMEI number and they'll disable it...

contact your provider by law now they have to do more to ensure the phone is not sold or cashed in and they can block the phone, not sure if it is all networks or just the one you are with though

phone who's network you were with and they will block it, then they can send you a replacement sim card for your new phone with your original number

You call your provider and they do it. I had my phone stolen and that's what you have to do.

Just thought that I would add that it cant be blocked unless you have insurance on it so unless you have that all they can do is block the sim card.

As well as telling your provider you can contact the NMPD (National Mobile Phone Database), or so I'm advised. (UK)
