It could be something with their mood such as they are tired, hungry, they have to go to the bathroom, their lonely, etc.

It's a glitch. It sometimes happens after sims take baths or showers, and sometimes it happens with the green stink that you see rising from them. Try having them dive again, or take a shower...have them do whatever caused the bubbles to come out of her head, and see if it goes away after a little bit. If not, try exiting into the neighborhood and go back into the household. If that still doesn't work then just try restarting the Sims. :P

I have never seen bubbles come out of head. tha is a little strange to me.
