I think John Wayne Gacy was pretty disgusting. Not the most gruesome but one of the most reprehensible.
Ted Bundy was a psycho.
Jeffery Dahmer may take the cake. He got what he deserved in prison.

Depends on your 'idea' of gruesome. Jeffery Dahmer raped and ate his young victims. Henry Lee Lucas cut his victims up like deer, and stuck their heads on stakes in his rooms. John Wayne Gacy buried 33 young men in a crawl space underneath his house. Ted Bundy sodomized and brutally beat his victims with a club. The canadian 'Ken and Barbie' murderers killed and raped teenage girls. If you're more specific in what you're looking for, I can give you more info... but 'Gruesome' isn't a lot to go on.
