There are no legal ramification to taking illegal narcotics during pregnancy because a fetus is not a person under the law. There is no point during pregnancy at which a fetus becomes a person under the law, he/she is only considered a person once they are fully extracted from the womb AND have drawn breath.

However if a baby is born WITH narcotics in his/her system the mother can be charged with child endangerment, etc, etc.

It's just as illegal to do drugs during pregnancy as it is when you aren't. The difference being, if you have a positive drug screen during pregnancy any other children you have could be removed from your home by social services. A home with drugs available is not a suitable/safe place for a child.
If you test positive when you are in labor - there is a good chance the baby could be removed from your care and placed in foster care.
The non-legal ramifications include deformaties, miscarriage, stillbirth. Cocaine and crack are especially good at causing extremely severe uterine contractions which basically blows the placenta off the uterine wall. Baby dies due to no blood supply, and mom could also die if they are not able to stop the bleeding from the placental site.
If you're worried about legal ramifications - I think you should ask yourself do you care about the baby. Is that hit of whatever you do really more important than your baby. There are plenty of places that counsel people regarding stopping drug usage. Check one out.

I'm a little embarrassed to admit, however My sister in law smokes pot. When she got pregnant they did all of the normal tests and found THC in her system. They advised her of this. She quit smoking pot when she found out she was pregnant however she smoked it for so long on a daily basis that on her second dr. appt it was detected again and she was told they would give her until her next appt to test neg for THC otherwise they would notify childrens services. I know this for a fact, she told me this herself and was quite embarrassed to admit it (as I would be to) Hopefully you are not asking this for yourself, it is a very sad situation and I feel bad for anyone that feels they have to be high, drunk or doped up some other way just to get through the day.
