Probably the only player who ever did this at the NBA level (if anyone did) was Wilt Chamberlain...however, steals and blocked shots were not official NBA stats during his career, subsequently, the NBA does not recognize such records prior to 1974, when blocks and steals became offical stats.

For example, in his first NBA game, Wilt had 43 points, 28 rebounds and 17 blocked shots (reporters kept track, but the NBA did not). The NBA does not recognize this game as a triple double for Wilt, even though it was.

But it's never been officially done...not in the NBA and not in NCAA men's basketball either.

As far as a quintiple double, that's nearly impossible, b/c there are only 5 categories to score in and if one is getting 10+ steals, it's very doubtful they are blocking shots and viceversa.

But I do know that David Robinson had quadruple double in the 90's, I don't know the exact stats but he did have 10 blocks and 10 assists with more than 10 points and 10 rebounds

Nate Thurmond, Alvin Robertson, Hakeem Olajuwon, and David Robinson in NBA, each had quadruple doubles only once. Nate, Hakeem, and David did theirs in pts, rebs, asst, and blks, while Alvin had his in pts, rebs, asst, and stls.

Wilt Chaimberlain may not have ever scored a quadruple double, but he is the only one every to double-triple-double in a game (22 pts, 25 rebs, 21 asst). He also had nine consecutive triple doubles.

There are no NBA quintuple doubles. The closest anyone has gotten to one in recent memory may have been Andrei Kirilenko of Utah Jazz who had 14pts 8rebs 9asst 6stl 7blks in a game against the Lakers.
