That's a question almost like "May Tom come out to play?" It would be a sentence if the words Tom and may were interposed. ie: Tom may go to work. (permission granted).

It could be either--depends where the punctuation is
May Tom, go to work. as in May Tom is a first and last name.
or
May Tom go to work?

both. sentences can be inquisitive or authorative or descriptive, or many other things, but it is still a sentence.
