That's a question almost like "May Tom come out to play?" It would be a sentence if the words Tom and may were interposed. ie: Tom may go to work. (permission granted).

It could be either--depends where the punctuation is

May Tom, go to work. as in May Tom is a first and last name.

or

May Tom go to work?

question because it have "may" in it. if any words has may in it that means it's a question.
