Easy one. If you look at worldwide figures for both sexes, WHO says smoking is the second major cause of death worldwide. Of the 650 million current smokers it says smoking will ultimately be the cause of half of their deaths!

Cancer over took heart disease as the biggest killer of men in the UK in 2003.
If you do not count Cancer as one group, then heart disease is the biggest killer of men, with Cerebrovascular diseases (problems with the blood vessels that supply the brain, eg Stroke) being second.
