I'm Canadian and I got mine in less then a month, you can also pay an extra fee to get it in 24 hours

Most post offices accept applications. You will need passport pictures, which you can get a Long's or Costco or any camera shop.

Some post offices do require an appointment, however.

If you are canadian, you have a long wait (2 months plus) If you are american or canadian, you can get an application at the post office, drug store, or go online for an electronic one. You can either mail it in, or go to the nearest passport office.

If it's a renewal you can print out the form off the net at - http://travel.state.gov/passport/forms/forms_847.html
get 2 pics at the autoclub or wherever & send in your old one with a check - they'll return both your old & new ones in about 6 weeks

well if you already have a passport and you need to renew it you can renew it in any postoffice the charge is 65 dollars. But if you dont have one any postoffice will get one for you in about 3 weeks for 140.
