Breastmilk, no, it shouldn't be heated in the microwave.
Formula I think the container says not to. I do know that overheating it does kill nutrients. Hot spots would be overheated and that would compromise it nutritionally.
Cow's milk is probably the same but at the age that a baby is consuming cow's milk as a breastmilk substitute(at least not before age one, and preferrably not before age 2) or not needing a breastmilk substitute it probably doesn't matter so much if a few of the nutrients are lost. Just as some nutrients are lost from cooked veggies, we still eat them.
However honestly it doesn't take very long to heat a bottle of liquid by placing it in a container of hot water, so what's the rush?

It doesn't kill the nutrients, but it doesn't heat very evenly, so there can be hot spots and cold spots in the liquid. This can be dangerous when your baby is eating from the bottle. It also is not good for the nipples; it deteriorates the rubbery susbtance in them, and over time, makes them sticky. I suggest placing the bottle in a pan of hot water or under a runninng faucet of hot water.

The doctors I work for tend to frown on it - only for the reason of uneven heating. There can be screaming hot spots and some cold spots and some perfect temperature spots, but because of the risk of burning a tender little mouth, they don't advise it.

You shouldn't need to microwave it in the first place, if it is cold or frozen allow it to sit in hot water or hot running water from the tap, it will warm it up sufficiently.

No, always use a bottle warmer or place the bottle in a jug or a bowl filled with hot water to warm it up
