It's any form of exercise that gets your heart rate up to a certain level, and keeps it there for at least 20 minutes, solid. The high level heart rate depends on your age, and other factors (example: mine is 120 -- my "resting" heart rate is 55).

Running, fast walking, jogging, treadmill, bicycling, aerobics, are all examples of exercise that gets your heart rate up.

Lifting weights would not be cardio, because your heart rate doesn't get "up there" and stay there for 20-30 minutes.
