Does he smoke? How about an engraved Zippo lighter.
Is he into jewellery? A nice curb chain.
Does he support a football team? You could have 18 and his name put on the back.
Does he have a car? A personalised tax disc holder.
Is he interested in cooking or any other hobby? You could buy him a book and write a nice message inside.
Does he read any magazines? A years subscription.

buy them jewellery (necklace or bracelet), a watch, pay for a tattoo, a book, photo album, photo frame with nice picture, digital camera, its hard to say whats something he will keep foreve,r there are many other great ideas but noone literally keeps them for the rest of their lives.. just giev him waht feels right and something that you think he will appreciate.

Watch, t-shirt, handmade craft (if u're a woman), sports jacket, shoes, or photo frame with both of your photo. Or u can buy anything he reall likes.
I bought my bro a watch for his 20th birthday, and he still uses it until now.
