Over the years, he has given several different stories on how he got his nickname. The most common one is that he stepped on the foot of his high school football coach, who, instead of swearing, shouted 'Meat Loaf!'.

another version is
Marvin Lee Aday's high school football coach in Dallas called him "Meatloaf" because of his size

there is also this one
according to his official website, his name started when his father called him "Meat" as a two-year-old. His schoolmates would later turn it into Meat Loaf as it has the same initials as his real name, M.L.

this one is the latest one I can find from a recent interview
Looking back on it now, do you think taking your stage name from a ground-beef dish might not have been the greatest idea?
This is what people don't understand. Anybody in their right mind would not have chosen that, okay? My father started calling me that when I was a kid. When I went to the first grade, my teacher said, ''Meat, sit down.'' I would go to church and the minister would say, ''Meat, we're glad to see you here.'' I can't win. But I don't think it's any funnier than other people's names. Start saying ''Bruce Springsteen'' over and over again. Everybody's name is weird.

I'm guessing because "small fry" wouldn't have been appropriate given his size.
