That is a good question...someone who is narcisstic is pretty self absorbed, and it takes someone fairly humble to look inward and realize that maybe they could afford to make some changes.
However, there could come a time when something happens to make them realize that maybe, just maybe they aren't ALL THAT!!!!!!!!!!!! and then they might be forced to look inside and make some changes. It is possible for anyone to change, given the right circumstances!
Life does offer some pretty humbling experiences for people, so I would say that there is a definite chance that the tide could turn at some point.

Well i think there is a little hope for that if not any hope at all! My best friend behaves like that and believe me a person so egoistic thinks of himself as someone superior an doesn't think that behaving like that is wrong. Also it would be possible that he has got this kind of life lesson from his parents so in that case things are a little complicated. Maybe with some cognitive-behavioral therapy. Unless of course they resist it and instead just debunk the therapist's credentials, far superior that they are.
Getting them into therapy is pretty difficult anyway because as others mentioned, they don't think anything is wrong with them. They are the perfect ones you know?
