are you asking from a practical or theoretical point of view? Theoretically it can have an ideal efficiency of 100%, but it would have to be frictionless. Energy is neither created nor destroyed, only transformed. So, to have an ideal efficiency, it would have to be frictionless as friction is a form of energy (or negative energy).

Because there is ALWAYS energy/ work lost to the environment. Commonly in the form of heat through friction. It is impossible prevent heat being transmitted off into the air/ surrounding environment.
