I have an .asp contact us form. I think it is pretty cool. You can use it if you are using asp.

This then sends the info to the asp file which then will email you or whomever you want.

Just remember to check them for security issues since nearly ANY FORM can be made to do bad things if the backend processing is not done well.

you can find any form you like from internet then copy and pase it to your html editor such as deamweaver, frontpage,even MS word will do.
