I suffered from the pain and aggravation of this for 2 years and saw about 5 urologists before I found one that had a solution.
The problem is, that area is very hard to get antibiotics into, and after you've been on 2 or 3 rounds of different antibiotics, your infection becomes resistant. Inflammation occurs and swells and traps the infected fluids, preventing circulation of antibiotic laden new fluids.
What cured me was 4 weeks, non-stop, of bactrim (it may have been biaxin but I can't remember. A dr. should have a good idea which would be the most appropriate), while also taking 'Hytrin'. Hytrin is a blood pressure medicine that will open up the blood vessels in that area and promote circulation.
During that time while you are on the meds, take long hot baths, avoid alcohol and coffee (because they are diuretics and tend to 'dry' you out), and make sure you take your antibiotics at the right time. Don't just guess and call it 'the morning and sometimes make it 6 am, and sometimes make it 10 am. Ditto if you take them at night. Space them out as evenly as possible.
Don't let yourself get run down during this time, get plenty of sleep, and hopefully your body and the meds will fend off the problem. Also, the prostate generates the same type of fluids as your sinus passages. Don't take decongestants/cold meds during this time frame. It will make you worse or prevent a cure. Many doctors will probably fail to remember or mention this, but it's true. Also, if you are into cycling, stay off the bike while you are on the drugs.
Also, ask the doctor for some high power anti-inflammatory medicine as well.
Supplement your diet with an herbal prostate mix of saw palmetto and pygeum blend, a cranberry concentrate called 'cranactin', and vitamins. Take 2 or 3 of the herbs and cranactin with each meal. Cranberries have been proven to make for a very inhospitable environment to bacteria in the u.t. area.
Bottom line is: get the drugs in there, control the swelling, and don't do anything to undermine your body trying to get well.

Something else to keep in mind: two of the favorite drugs for this are cipro or levaquin. These are in the flouroquinolone family. They have the nasty side affect of leaching calcium from your load bearing tendons in some people and causing strains and crippling, in addition to causing neural damage. Alot of Doctors and the drug companies will deny this as a problem, but I suffered it myself. If you take either of these drugs and find that you are having tendon pain or nerve issues, stop immediately. This is not a side affect. It's a toxic reaction to the antibiotic and can have very, very long term side affects that don't just go away when you stop the drug. One pill can be a problem if you are sensitive to it. If your Doctor denies any of this, he is simply uniformed. Sadly, many of them don't know this information, and continue to damage people after years of clinical and AMA research proving the problems. Many of the urologists I saw were less informed than I was, simply because I had an intense interest in getting well and found that if I was going to get well, I would have to do it myself. Also, instead of trying urologists, see if you can find a men's clinic that specializes in problems of this sort.
There's a good one in North Dallas, but I have no idea where you are from. I can tell you this: in hindsight, I would have flown in from siberia if I had known that one outfit could have cured me before 2 years of hell and a bunch of urologists that couldn't solve the problem.
Good luck. You can get cured, but you've got to be very, very serious about it for about a month.

I hope that at this stage you have been to an urologist..... If not get an appointment immediately. This is ridiculous... tell the doctors so... You're the boss not them. They say that drinking cranberry juice is soothing.
