Except for your family, employers could care less about Junior, so delete it .

Definitely, First, Middle, Last. Keep out the Jr. That is very yesterday and almost looks as if you can't make a name on your own.

First M. Last, and leave out all naming suffixes like Jr., II/III, esq. Unless ur a medical doctor or a PhD, it shud just be your name.

use the name that is on your legal documents. Saves problems if you start the job and they want proof of your name, tax etc.
