I believe this happened five or six years ago when there was a new conference created. I don't know which conference it was, but their champion had to get an automatic berth, and I'm guessing the committee didn't want to take away a spot from a more deserving at-large team, so they just made it a play-in game.
On a slight tangent, the play-in game is crap. Two small schools win their conference tournament for the right to go to the Big Dance, but only one school actually gets there. The other school gets to say they were eliminated in the play-in game by East Western South Central Northeastern State Tech. Some reward for a great conference tournament run...

At one point there were 33 conferences, only for a year or two. Now that the number of conferences as gone down, they still kept the extra team. The pig-tail game (normally between the 64th/65th ranked teams) decides who plays against the best number one. The game is normally played at a smaller location like Dayton, Ohio.
