It would depend upon its implementation. Yes VoIP can be HIPAA compliant.

Do not assume that all VoIP systems use the internet as a transport mechanism. Even if it does, using VPN and encryption it can be made secure. Plenty of medical information already goes over the internet, why not voice.

So you are asking if a technology designed for making voice calls over the internet is compliant with a regulation designed to protect the privacy of personal health information?
