What are the rules/regulations for botox providers?

What type of insurance do they need? Are there other restrictions on the service? Do any states have specific regulations regarding any of the medi-spa type treatments?
I'd prefer information from someone in the business if at all possible.

I'm not sure about the insurance or necessary paperwork. All I can find says "check with the regulatory agency in your state" but the agency has no data. Good luck.
