Well I think it depends. If you have been a long relationship then face-to-face. Otherwise, a phone call should do.
"How" to leave? Well if he is prone to violence (you should already be gone), but do it while he's at work. I DID!
If he's smart I'm sure he knows it over anyways.

Your suppose to move where? Will you still be seeing him where you move?
Maybe you should just be honest on this one and tell him that you've enjoyed the time you spent together with him and that it ends when you move out. You can tell him over the phone, in person or what your most comfortable with. Put yourself in his shoes, how would you want someone to end it with you? Look at it from all views and then apply it to your own creativity but be honest enough so that it doesn't haunt you later. It is a small world and you could bump into him later in life. Good luck!

I haven't seen your other questions but I do know if you are leaving you should tell your boyfriend to his face the relationship is over and you are moving out if you really mean it. If you are afraid of him then maybe you should just leave.

If he has anger problems don't tell him in person.
But if he doesn't have anger problems or your not scared of him then I'd tell him in person.
Make it simple and to the point.
Tell him the reasons why.
and then leave.
