Phenol is a solid with a melting point of 40.5 °C.

The chloroform and isoamyl alcohol are the solvents for this solution. The Phenol has come out of the solution and condensed on the sides of the glass bottle.

All you need to do is agitate (swirl) the contents of the bottle until the Phenol redissolves (before you open the bottle).
