See if she has a good baby carrier (not the average Snugli front carrier). Having a second baby is often more overwhelming than having the first, because you are not only dealing with newborn needs 24/7, but also have an older child to look after and play with, pick up after, etc.
One of the best gifts I like to give is a sling carrier, which I found indispensable with my babies, especially since I believe in holding them as much as they want to be held. So they got held a LOT. ;) But whenever I've given someone a sling, it has been just as well-loved by them. It's also a fun excuse to get together shortly after the baby is born, because slings can be somewhat tricky to learn to use (simple, but tricky, if that makes any sense), so I always offer to give a good demonstration to get them comfortable using it.
Slings run anywhere from $20- over $100; average being somewhere between $30-40 I believe.

Since she essentially has what she needs for the baby, I would suggest two things: a baby book for the new baby because she will actually need that and cannot use what she already has; or, and this is my favorite, a visit to a day spa for the mom. She can use it pre or post baby, but it is something that will be just for her. And something she will need. Oh yes, don't forget to take a small gift for the big sister so that she will not feel over-looked. Just something small but wrapped for her will do. Good luck.

I got a really neat present from a friend who was tight on money and the presentation of it was the best even though it didnt have alot in it.
She took a long piece of ribbon and clipped several onesies, a bib, a pair of socks, 3 birb clothes, and a coupon for a free night of baby sitting. She had clipped them onto the ribbon with clothes pins so that when I pulled out the ribbon all of the items came out and it looked like a clothes line. Each items was clipped on individually so it makes it look like more.
I thing she spent about $20-25 on the whole gift.

When my daughter (my first) was born I wish I had a good baby book to keep all the memorys in. Also, diapers and wipes will always be help full. Soaps and lotion will mike a nice gift basket. Also Clothes are need even if she has hand-me-downs, something new and different will to nice too.

lol okay this is really going to sound like a really silly gift, but I ended up needing and loving it..
Baby mittens. My beautiful baby will scratch her own eye out of her face if i let her. they are very inexpensive, and then you could get a larger gift.
The second gift is diapers.. you never really have enough.. Even if she is well off, who wants to leave the house just to get diapers.
Check if she has a registry tho.. you can usually find a gift that she really wants.
P.s. I almost forget the best gift i recieved was a smooth motions glider. my little baby drifts off to sleep land when she sits in it for more lets see um .02seconds lol and that price is about $60.00. good luck and have fun.

I just had my second girl and to be honest no one thought of me! You are special thinking about your sister in law!
To be honest I would suggest to make up free tickets like for example 1 free night to go out for dinner with your husband, 1 free afternoon where you can do anything you want while you watch the children, 1 free house cleaning, 1 free laundry free day and so on.
Another suggestion that I can make is make some dinners and deserts that can be frozen and used later!
One last suggestion is to give her a gift card to a place that develops digital photos (that is if she has a digital camera)

The best gift for a person that has everything....something that gets better with time. Go purchase a $25.00 savings bond for the child. It's sure to be a winner gift. Insert in a beautifully decorated card. Be creative, personalize the card, with the baby's last name.

If she has everything she needs the why don't you get HER a gift card to get her nails done or a pedicure and tell her that you will keep the baby for an hour or two while she pampers herself she will really like being able to take some time for herself.

I would get a little basket that you can buy at wal-mart that has a blue or pink bow on it. Fill it with the necessities that people don't usually get at a baby shower. Like the outlet plug things, Some diapers (about a size 2 because people usually get newborn diapers)....just things like that.

I have twins and the thing that gets me through the day is this cheap little fischer price glider/rocker chair. Its great. Its really cheap..around 20 bucks. Its blue and its got a cow and a barn on it. It is perfect for rocking them with your foot when you are busy doing something else. I take it room to room so they can watch me do whatever im doing. And even now that they have started solids and are not ready for the high chair..i feed them in that thing..best 20 buck i ever spent!
