Because it dominates your thoughts or mindset, i.e your head.
A metalhead constantly thinks about heavy metal; it's what his life is about. Therefore, metalhead.

good thought for the day. I'm gonna have fun with this one. i must be a petrol head coz I'm an f1 fanatic but because I'm also a fan of food, could i be called a fat head. i like to sleep so i could be a bed head
