me and my wife want a fresh start and we both want to move to cornwall and we dont now were to start can any one help us please

Cornwall is a high demand area and so the waiting lists are long, 5 years is not uncommon.
You have one big advantage by being a social tenant. This will help you transfer. Don't give it up to move!.
Try advertising in the local papers in the area you want to move to for a swop. This will be the quickest route. If no luck at first, widen your area. People are always on the move so you should be lucky.
Ask councils and housing associations if they have a mutual exchange board, you will be able to advertise your home in Stoke there and for free. Sell it well with a nice photograph, listing closeness to shops, public transport, parks etc.
To finish on a word of encouragement, I found a mtual exchange from a tower block in Manchester to a quiet, country flat in the best part of Dorset in 3 weeks. I love it here.
Also advertise a move in LOOT, they have a very good section for mutual exchanges. You can get LOOT in Stoke.
Approach all councils and housing associations in Cornwall but to be considered you will need a job in the area, family connection, or living there.

advertise in the cornwall local papers for an exchange to stoke on trent. describe your property and say what you are after. If you have a 3 bed and are willing to give it up for a two be you may have more hope. Also ask council if they have any scheme's for moving to annother area.

Why not go to your local council and ask if there is an exchange from stoke on trent to cornwall

There is no quick way, i live in stoke-on-trent the council need a rocket up their a**e before they do anything. The best thing you can do is speak to your local housing office
