You can't have a nephew twice removed, a different terminology is used. Removed means a generation. Here's how it works.

Your parents have brothers and sisters. Their children are your first cousins. The children of your first cousins are your first cousins one removed, their children will be your first cousins twice removed.

Your children will be the second cousins of your first cousins one removed and your grandchildren will the the third cousins of your first cousins twice removed.

First cousins share grandparents. Second cousins share great grandparents. Third cousins share great great grandparents. Any relationship up or down requires a removal.

Your nephew's children are your great nephews and their children are your great great nephews. Your nephew is your children's first cousin. Your nephew's children are your children's first cousins once removed.

It is easy to sketch it on paper and you can see the relationships and removals more clearly.

"Twice removed" refers to two generations away. For example, suppose you have a first cousin who has a child. That child would be your first cousin, once removed. If that person then had a child, he or she would be your first cousin twice removed.

So a nephew twice removed would be the equivalent of your great great nephew.

A sibling's grandchild is either a grandniece or a grandnephew. They are removed two generations because they are the children of a sibling's child.
